What is an Individual Support Package?

  • Require on-going disability support
  • Have your need for support recorded on the Disability Support Register

Contact DHS to find out if you are eligible for ISP funding.

What is Individual Support Package Facilitation?

Our team can help you to develop and ISP plan and put it into action. We want you to make an informed decision on the types of support you need and how we can provide it for you.

The role of the ISP facilitator is to give you advice and guidance. Often, we involve your family and carers in the planning process. Together, we can help you find the support you need.

ISP facilitation is currently only available in Eastern and Hume areas.
