Scope values the perspectives of those who love and care for our clients – their families and supporters.

Scope is undertaking a survey of all family members and supporters of clients receiving services at their Supported Independent Living and Social Connections sites.

The survey will provide an opportunity for you to give feedback relating to your loved one’s service expectations and experiences.

You will receive an email or SMS with the survey link, or a phone call to complete the survey depending on what contact information you have given to Scope.

The survey will be open until 6th November.

A few things to note about the survey:

  • Your responses will be de-identified and collated with other survey responses to provide broad themes and insights down to the service site level, unless you ask us to specifically follow up on your feedback or your feedback raises concerns around the safety of a client
  • To find out more about how we keep your information safe, please see our privacy policy
  • It should take no more than 15 minutes to complete, depending on how much detail you want to give us
  • The link we will send you is unique to you. Please do not share it with others. If you have more than one family member or supported person using a Scope service, you will receive a unique link for each person
  • You can choose to complete a survey in relation to any or all such persons. If your loved one or supported person is supported by Scope for both Supported Independent Living and day services (Scope Social Connections) you will only receive one unique link.
