Alba

Alba is 24, works part time in supported employment and would like to live independently. She is exploring funding options as at this stage she does not have the funding to explore options outside the family home. The good news is she has been funded for a Support Coordinator.

Alba began looking for a suitable Support Coordinator by asking questions about their experience in independent living, ILO and supported accommodation, supported independent living and specialist disability accommodation. Alba found Support Coordinator Lisa who had experience in this area, who was able to support Alba to connect with suitable allied health professionals to support her goals. Tom is 10 years old and has just received an autism diagnosis and his first NDIS plan, which included Support Coordination. Tom’s parents feel overwhelmed and do not know where to start. They identified three main areas to focus on were communication, self care and behaviours of concern.
